Care Navigation

 

You may have noticed that we now ask for a brief outline of your problem when you call to make an appointment at Daneshouse Medical Centre. This is because we have developed a new role in the practice - called a care navigator - to help you see the right healthcare professional first time. We have trained a number of staff to take on this new role. Our care navigators will work with you to fully understand what your needs are and ensure that we help you with your problem efficiently and conveniently.  Our care navigators never offer clinical advice or triage; this is about offering you the choice to see other more appropriate health professionals, often quicker and without the need to see the GP each time.

By working this way, it helps us to free up time for the GP's to care for our patients with complex or serious health conditions and it means that you will find it easier to get a GP appointment when you need one.

Changes to Prescription Ordering

Please note that from the 1st October 2017 pharmacies will not be allowed to order on the behalf of a patient.

A request for medication can be made in surgery, via the online patient access service or via fax. 

Patient Self Care

The local clinical commissions group for East Lancashire carried out engagement with local GPs and local residents about the prescribing of medicines for minor ailments.

Around 824 people took part in an engagement surgery, and there was overwhelming support that we should stop funding these types of prescriptions. From the 1st October 2017 these items, including the list below, will no longer be routinely prescribed.

  • Pain killers for minor aches and pains

  • Tonics, health supplements and vitamins

  • Earwax removers

  • Hair removal creams

  • Moisturisers and sun cream

  • Antihistamines for hay fever/bites

  • Head lice treatment

  • Lozenges, throat sprays, mouthwashes, gargles and toothpastes

  • Indigestion remedies for occasional use

  • Preparations for infant colic, nappy rash or teething

  • Creams for bruising, tattoos and scars

Named GP

What is the named GP requirement?

This is a contractual requirement and builds on the 2014-2015 agreement to provide a named and accountable GP for over 75s. The named GP requirement has now been extended to all patients. 

By 31 March 2016 all practices will need to include on their website reference to the fact that all patients, including children have been allocated a named, accountable GP.

What does 'accountable' mean?

The contract requires the named accountable GP to take responsibility for the co-ordination of all appropriate services required under the contract and ensure they are delivered to each of their patients where required (based on the clinical judgement of the named accountable GP).

The contract remains 'practice based', so overall responsibility for patient care has not changed. This is largely a role of oversight, with the requirements being introduced to reassure patients that they have one GP within the practice who is responsible for ensuring that this work is carried out on their behalf. 

Does the requirement mean 24-hour responsibility for patients?

No. The named GP will not:

    • take on vicarious responsibility for the work of other doctors or health professional
    • take on 24-hour responsibility for the patient, or have to change their working hours. The requirement does not imply personal availability for GPs throughout the working wee
    • be the only GP or clinician who will provide care to that patient

Will practices have to write to patients to tell them their named GP?

There is no requirement to write to any patients regarding their named GP. Practices are required to inform patients of their named GP at the next appropriate interaction and it is for practices to decide what is appropriate.

What does this mean for me as a Daneshouse Medical Centre Patient?

You have a named GP. Your named GP will ensure that you receive the best possible level of care and support from our Surgery and will work with other relevant healthcare professionals, who are involved in your care, to ensure that your care meets your individual needs.
